Verification documents must be provided before enrolling into the CBE course.
You are required to show proof that you meet ALL of the below requirements:
- You are a registered healthcare professional with a license in good standing with a regulatory body.
- You must have completed the Introduction to Obesity Course.
- You must have a minimum of 700 hours of practical obesity experience within the last 5 years. A letter from your employer is required as verification that you have the practical experience.
You are responsible for ensuring your documentation is submitted correctly before you purchase the course. If you do not adhere to the submission requirement you will not be eligible to write the exam.
Verification documents MUST follow the below format when submitting:
You must combine ALL required documents into ONE PDF file.
Your file must be renamed to the following format:
